Our Vision

Meadowbrook’s Brain Injury Rehab is for individuals who have suffered a traumatic or acquired brain injury who have the goal of returning to a community setting. These individuals may also have complex medical needs that complicate their rehabilitation process. The purpose of our program is to enable them to reach their highest level of independent function and quality of life.

Meadowbrook TBI

Rehabilitation
Process

Patients are expected to participate, to the best of their abilities, in the rehabilitation program, their plan of care, and their discharge planning process. Family involvement is also highly encouraged and vital to patients’ success in the program and discharge. Family meetings, which consist of the individual, the family, and the insurance case manager are held regularly to ensure the patient is showing progress towards their rehabilitation goals. Interdisciplinary team conferences are held every two weeks to discuss the patient’s progress, barriers to discharge, medical status, and discharge needs. Each patient has a designated social worker, who acts as a liaison between the patient, the family, the payor and the treatment team.

Meadowbrook TBI

Rehabilitation Outcomes

The length of stay for patients can range from a few months to over a year. The length of each patients’ stay will depend on the individual’s level of functioning, medical status, discharge plan, and level of support at the time of discharge. Meadowbrook’s focus is on helping our patients and their families navigate their ‘new normal’ to a lifestyle that works for them, in a safe, appropriate environment. We work with several local agencies to assist in the arrangement of services needed to provide a smooth and seamless discharge back into the community.
